Job summary

Family Reach is seeking a Resource Navigator to deliver remote resource navigation services to people facing cancer. You will manage a caseload of patients and caregivers, connecting them with essential resources such as food, housing, utilities, and transportation to ease financial stress during treatment.

The role requires knowledge of social services, health insurance landscapes, crisis management, and strong communication.

Qualifications

• 2–4 years of professional work experience, especially in oncology.
• Excellent oral and written communication skills.
• Proficiency in technology suited for remote work.
• Highly organized and detail-oriented.
• Ability to be both collaborative and independent.
• Solutions-oriented; eager for ongoing learning and self-improvement in the role.

• Access to a private computer/laptop.

Responsibilities

• Provide navigation to critical resources to support basic needs for patients and caregivers in a remote environment.
• Operate with compassion, respect, and empathy for each patient.
• Assess needs in food, housing, utilities, and transportation.
• Identify eligible resources and facilitate connections.
• Manage a caseload using electronic documentation and records with high data accuracy.
• Conduct intakes or assessments over the phone and share information about Family Reach's Financial Resource Center.
• Update referrals statuses and track outcomes to demonstrate program effectiveness.
• Collaborate across departments and participate in weekly team meetings to share insights and improve Navigation services.
• Support cross-functional initiatives and special projects as needed.
