Job summary

United States Digital Space LLC is seeking a Community Resource Navigator to connect members to community resources and support their care management journey. The role helps the care team identify community supports and deliver health education as appropriate.

This position offers remote work from anywhere within Missouri, with a Monday–Friday schedule and compensation in line with market rates for community health workers.

Qualifications

• Requires a High School diploma or GED.
• 1–2 years of related experience.
• Ability to work remotely within Missouri.

Responsibilities

• Provides support to connect members to community and care resources cost-effectively.
• Coordinates community outreach resources and promotes awareness of care/services.
• Supports inquiries and opportunities related to community and care resources.
• Assists with member correspondence and educational materials for care connection.
• Documents and maintains community resources per health plan policies.
• Assists nurses and social workers with outreach, education, and member support.
• Conducts non-clinical health assessments to refer members to care/services.
• Performs non-medical assessments (home safety, environment, transportation, employment) for referrals.
• Conducts telephonic and in-person outreach to locate hard-to-reach families.
• May make visits to homes or community organizations.
• Demonstrates knowledge of Social Determinants of Health (SDOH) barriers.
• Performs other duties and complies with policies.
