Job summary

HF Management Services, LLC seeks a Relationship Coordinator to educate prospective Healthfirst members about Medicare Advantage and long-term care programs. The role handles inbound/outbound calls, expertly navigates Salesforce and related systems, and supports enrollment through timely follow-up in a paperless call center.

You must be fluent in English and Spanish, with customer service experience, and able to work remotely with reliable internet.

Qualifications

• High School diploma or GED required.
• Fluent in reading and speaking English and Spanish.
• Customer service experience in healthcare or related field.
• Able to work 8.5 hour shifts within operating hours, including open enrollment.
• Capable of remote work with reliable internet connection.
• Proficient with Microsoft Office and common software tools.
• Organized, detail-oriented and able to multi-task.
• Sensitive to cultural differences and community needs.
• Able to work under pressure to meet quotas and goals.
• Comfortable communicating with leaders via virtual platforms.

Responsibilities

• Handle inbound and outbound calls to address customer needs within SLAs.
• Create and follow up for LTC Leads and Opportunities in Salesforce for up to 90 days.
• Create and follow up for Personal Care services requests for Mainstream population in Salesforce (up to 30 days).
• Answer calls or execute outbound calls per approved scripts.
• Search Medicaid eligibility and CMS eligibility codes in ePACEs and Marx.
• Meet daily, weekly, and monthly growth goals.
• Schedule field and tele-video appointments for SHP Intake Nurses and HF CompleteCare teams in Salesforce.
• Document all interactions in Salesforce and pre-enrollment databases.
• Coordinate fulfillment materials and respond to information requests promptly.
• Engage with State brokers and health plans to support enrollment processes.
• Troubleshoot LTC enrollment rejections and perform other duties as assigned.
