Healthfirst is seeking a Relationship Coordinator to educate potential members on MAP, MLTC SHP, and HARP programs, guiding them through LTC enrollment and Medicaid eligibility checks. You will support a paperless, security‑driven environment, using Salesforce and Office 365 to manage leads, appointments, and outcomes.

You will handle high-volume calls, pursue daily/weekly/monthly growth goals, and operate in a remote setting with flexible hours, including weekend shifts during enrollment

Qualifications

• High school diploma or GED is required.
• Fluency in English and Spanish is required.
• Experience in customer service or healthcare-related calls is preferred.
• Ability to work flexible hours including weekends during enrollment.
• Experience with computerized databases and remote work tools is a plus.

Responsabilités

• Handle inbound and outbound calls while meeting Service Levels standards.
• Create and follow up on LTC leads and opportunities in Salesforce for up to 90 days.
• Schedule appointments for SHP Intake Nurses and HF CompleteCare staff in Salesforce.
• Document calls, outcomes, and instructions in Salesforce and pre-enrollment systems.
• Coordinate fulfillment kits and information materials for prospective members.
• Engage with leaders via virtual platforms and strive to meet daily, weekly, and monthly goals.
• Support the enrollment process for Medicaid/long-term care services and eligibility checks in CMS systems.
