Ramey-Estep Homes, Inc. is seeking a Care Navigator to serve as the central contact for individuals accessing behavioral health, primary care, and community supports.

The role blends in-person engagement with remote telehealth and administrative duties to ensure timely, person-centered care aligned with CCBHC standards. The position requires greeting clients, screenings, intake, and education about eligible services, supports continuity of care, and coordinates with clinicians and community

Qualifications

• High School Diploma or equivalent required.
• Associate’s or Bachelor’s Degree in healthcare, social services, or related field preferred.
• Knowledge of community resources, health systems, and behavioral health navigation.
• Experience in behavioral health, case management, or community navigation preferred.

Responsabilités

• Welcome and support clients in-person, via phone, or telehealth.
• Conduct initial screenings to determine eligibility and service needs.
• Complete intake forms, consent, and release documents (HIPAA/42 CFR Part 2 compliant).
• Educate clients on available services, rights, grievance procedures, and community resources.
• Connect clients to behavioral health, primary care, and community support services.
• Track and follow up on referrals to ensure successful connection.
• Collaborate with clinical and case management teams on individualized care plans.
• Document client interactions, progress notes, referrals, and care plan updates in the EHR.
