Job summary

Elevance Health seeks a Behavioral Health Case Manager I to perform telehealth and home visits within licensed scope of practice for members with behavioral health or substance use needs.

Role emphasizes assessment, care planning, and coordination with providers, payors, and community resources to ensure optimal, cost-effective care delivery. This is a hybrid/remote role with in-person training requirements.

Qualifications

• Requires MA/MS in social work, counseling, or related behavioral health field; nursing degree acceptable with relevant clinical experience.
• Current active unrestricted license (RN/LCSW/LICSW/LPC/LMFT/LMCS etc.) per state law and scope of practice.
• Experience with complex psychiatric and substance abuse cases is preferred.

Responsibilities

• Assess member needs using screening criteria and clinical judgment.
• Develop and monitor care plans; adjust as needed to achieve objectives.
• Coordinate with internal/external resources and providers to ensure quality, cost-effective care.
