Job summary

Elevance Health is seeking a Behavioral Health Case Manager to perform telephonic case management and occasional home visits for members with behavioral health and substance use needs. The role requires licensure and 3+ years of relevant clinical experience, with hybrid in-person and virtual work options.

Responsibilities include assessing needs, developing care plans, coordinating with providers, and monitoring outcomes to ensure access to appropriate care.

Qualifications

• MA/MS in social work or nursing degree or equivalent.
• Active unrestricted license (RN/LCSW/LICSW/LMHC/LCPC/LMSW or Psychologist) as required by state law.
• Experience with complex psychiatric and substance use disorder treatment.

Responsibilities

• Assess member needs using screening criteria and clinical judgment.
• Develop and monitor individualized care plans addressing identified objectives and goals.
• Monitor care plan effectiveness and modify plans as needed; ensure access to quality, cost-effective care.
• Coordinate with internal and external resources and collaborate with providers.
