Job summary

Elevance Health is hiring a Behavioral Health Case Manager I to perform telephonic case management and/or home visits within licensed scope for members with behavioral health and substance use needs. The role emphasizes flexibility, autonomy, and compliant hybrid work arrangements.

The position requires licensure and a minimum of three years of relevant clinical experience in social work, counseling, or nursing, with a focus on complex psychiatric and substance use treatment.

Qualifications

• Requires MA/MS in social work, counseling, or nursing with minimum 3 years of clinical experience in social work, counseling, or related behavioral health field.
• Current active unrestricted license such as RN, LCSW, LICSW, LMHC, LPC, LMFT, or Clinical Psychologist to practice within licensure scope.
• Background that permits practice as a health professional within applicable state laws.

Responsibilities

• Assess member needs using screening criteria and clinical judgment.
• Develop and monitor care plans to address identified objectives and goals.
• Coordinate with internal and external resources to ensure access to appropriate, quality, cost-effective care.
• Collaborate with providers and case management teams to support members' behavioral health needs.
