Job summary

Elevance Health in Virginia seeks a Behavioral Health Case Manager I to assess member needs using clinical judgment, develop and monitor care plans, and coordinate with providers for effective, affordable care. This hybrid/remote role includes in-person training and weekly onsite expectations as part of Elevance Health’s Hybrid Workforce Strategy.

The position requires an MA/MS in social work or related field or nursing degree with at least 3 years of clinical experience, plus licensure

Qualifications

Minimum of 3 years clinical experience in social work counseling with broad range of complex psychiatric and substance abuse treatment.
Current active unrestricted license such as RN LCSW LMHC LICSW LPC LMFT LMSW or Clinical Psychologist to practice as a health professional within applicable states.
Candidates must meet licensure requirements per state law and scope of practice.

Responsibilities

Uses screening criteria knowledge and clinical judgment to assess member needs.
Conducts assessments to identify needs and develops care plans to address objectives.
Monitors and evaluates effectiveness of care plans and modifies as needed.
Coordinates with internal and external resources to meet identified needs and collaborates with providers.
