Job summary

The Elevance Health Companies, Inc. is seeking a Behavioral Health Case Manager I to assess member needs, develop and monitor care plans, and coordinate with providers and resources.

This remote-friendly role supports flexible scheduling with required in-person training sessions. Candidates should have an MA/MS in social work or counseling (or nursing) and at least three years of clinical experience in behavioral health, with active licensure where required.

Qualifications

• Requires MA/MS in social work, counseling, or related behavioral health field, or nursing degree.
• Minimum 3 years of clinical experience in social work or counseling with diverse psychiatric/substance use treatment.
• Active unrestricted license such as LCSW, LMHC, LPC, LMFT, LMSW, or Clinical Psychologist per state law.

Responsibilities

• Assess member needs using screening criteria and clinical judgment.
• Develop and monitor care plans to address identified objectives.
• Coordinate with internal and external resources to access quality care.
• Support access to cost-effective services and track plan effectiveness.
