Qualifications
- MA/MS in social work, counseling, or related behavioral health field or nursing degree.
- Licensed to practice in Virginia (LCSW/LPC/LMFT/Clinical Psychologist as applicable).
- Minimum 3 years' clinical experience with psychiatric and substance use treatment.
Responsibilities
- Perform case management telephonically and via home visits.
- Assess member needs and develop care plans to address goals.
- Coordinate with internal and external resources and providers.
- Monitor care plan effectiveness and modify as needed.
