Job summary

UnitedHealth Group is seeking a Wellness Coordinator to provide non-clinical care coordination and administrative support for behavioral health and substance use needs. The role includes engaging members, identifying gaps in care, and coordinating with clinical partners to improve access to services.

The position is remote nationwide with a 40-hour week, CST schedule, and comprehensive benefits. Prior experience in behavioral health and strong communication are essential.

Qualifications

• 2+ years of experience in behavioral health and case management.
• 1+ years of computer proficiency experience.
• 1+ years of dual diagnosis experience.
• 1+ years of handling sensitive issues with members and providers.
• Access to a home workspace with ability to secure PHI.
• Live in an area with UnitedHealth Group approved high-speed internet.

Responsibilities

• Handle inbound and outbound member queues.
• Document in multiple electronic systems.
• Coordinate information with clinical team partners.
• Provide behavioral health benefit education.
• Screen for and identify gaps in care with members.
• Provide network referrals and community resources.
• Provide administrative support.
• Perform other duties as assigned.
