Job summary

UnitedHealth Group is seeking a Care Coordinator to serve as the primary care manager for dual eligible members, focusing on medical, behavioral, and socioeconomic needs. The role supports a person-centered approach and requires strong organization in a fast-paced environment.

Qualifications

• Idaho license in one of the listed clinical licenses.
• 1+ years of experience with Medicaid/Medicare or SDoH needs.
• 1+ years of MS Office (Word, Excel, Outlook).

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Conduct comprehensive needs/wellness assessments and develop person-centered care plans.
• Collaborate with care team, providers, and community resources to implement plans.
• Assist members with HCBS support and services referrals.
• Coordinate care transitions after ER visits or SNF stays.
• Educate and coach members for self-management of care and lifestyle changes.
• Advocate for members to ensure needs are represented by the care team.
• Address nonclinical questions and connect providers to the right Health Plan resources.

LPN/LVN

2-year healthcare degree or higher
