Job summary

Sentara Healthcare, Inc. seeks an Advanced Practice Provider to deliver virtual care after hospital discharge, addressing immediate clinical needs and coordinating transitions to primary care and other services.

The APP reviews discharge records, manages lower-risk conditions, coordinates referrals, and ensures clear handoffs and documentation. This is a 100% remote role with flexible weekday/weekend hours, including a closing shift and no on-call requirements.

Qualifications

• APP licensure and credentials appropriate for the assigned service area.
• Minimum of five years of clinical APP experience (family, emergency, or long-term/post-acute care).
• Strong communication, organization, care coordination, and independent decision making.

Responsibilities

• Review discharge records and complete focused assessments and medication reconciliation.
• Manage lower-risk diagnoses and chronic conditions within program scope.
• Recognize red flags and escalate to higher care using protocols.
• Coordinate referrals and services (home health, social work, etc.).
• Complete timely documentation and clear handoffs for tests and follow-up.
