Devoted Health is seeking an experienced Advanced Practice Provider (APR N/PA) for the Veterans TOC program. The role combines initial post-discharge visits with virtual urgent-care and chronic-care management, within a highly interdisciplinary team.

The ideal candidate has 5+ years in geriatric care, strong acute and chronic disease management skills, and licensure or eligibility to practice in required states within 90 days of hire. This is a full-time position with strong total rewards.

Qualifications

• 5+ years direct patient care with geriatrics and multiple chronic conditions.
• Experience across acute and chronic care continuum.
• Active nursing or medical license with eligibility in required states within 90 days of hire.

Responsibilities

• Perform comprehensive initial TOC visits and post-discharge assessment.
• Conduct acute/virtual urgent-care TOC visits and manage symptoms.
• Coordinate 30-day care trajectory with care teams and follow-up.
• Participate in interdisciplinary TOC rounds and quality-improvement projects.
• Contribute to EHR enhancements and clinical education.
