Job summary

Sentara Health in Virginia Beach seeks an Advanced Practice Provider to deliver timely, virtual care to patients after hospital discharge. The APP will review records, perform focused assessments, reconcile medications, and create short-term care plans.

This remote, full-time role requires five years of APP experience in family or acute care, strong communication and care coordination skills, and the ability to work independently within protocols. VA/NC licenses may be required.

Qualifications

• APP licensure and credentials for the service area.
• Minimum of five years of APP clinical experience in family medicine, emergency medicine, or long-term/post-acute care.
• Strong communication, organization, care-coordination, and independent decision-making.

Responsibilities

• Review discharge records and complete focused assessments, medication reconciliation, and time-limited care plans.
• Coordinate referrals and services, including home health, wound care, social work, behavioral health, pharmacy, and durable medical equipment.
• Complete timely documentation, continuity-of-care paperwork, and clear handoffs for tests, referrals, and follow-up needs.
• Manage appropriate lower-risk diagnoses and chronic conditions within program scope.
