Job summary

Sentara Healthcare, Inc. seeks an Advanced Practice Provider to deliver virtual care after hospital discharge, addressing immediate clinical needs and coordinating transitions to primary care and other services.

The APP reviews discharge records, manages lower-risk conditions, coordinates referrals, and ensures clear handoffs and documentation. This is a 100% remote role with flexible weekday/weekend hours, including a closing shift and no on-call requirements.

Position summary: The Advanced Practice Provider will deliver timely, virtual care to eligible patients following hospital discharge. The APP will address immediate clinical and coordination needs, support a safe transition, and provide a clear handoff and/or bridge to primary care and other appropriate services.

Key responsibilities:

Review discharge records and complete focused assessments, medication reconciliation, and time-limited care plans.

Manage appropriate lower-risk diagnoses and chronic conditions within program scope.

Recognize red flags and escalte to emergency, specialty, in-person, or higher-acuity care using established protocols.

Coordinate referrals and services, including home health, wound care, social work, behavioral health, pharmacy, and durable medical equipment.

Complete timely documentation, continuity-of-care paperwork, and clear handoffs for tests, referrals, and follow-up needs.

Preferred qualifications:

APP licensure and credentials appropriate for the assigned service area.

Minimum of five years of clinical experience as an APP in family medicine, emergency medicine, or long-term/post-acute care, including the management of medically complex adults and focused virtual assessments.

Strong communication, organization, care-coordination, and independent decision-making skills.

• Will be required to obtain a VA and NC License
• Applicants may live in the following states: Indiana, Nevada, South Carolina, South Dakota, Tennessee, Texas, Virginia, West Virginia, or Wisconsin

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $100,900.80 - $312,000.00 annually. Additional compensation may be available for this role, such as shift differentials, on-call pay, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.
