Sentara Health is seeking an Advanced Practice Provider to deliver timely, virtual care to eligible patients following hospital discharge. The APP will address immediate clinical needs, support a safe transition, and hand off to primary care and other services.

This 100% remote position offers flexible hours, a closing shift, and the potential for incentive compensation. The role focuses on acute issues and chronic condition management, with no call requirements and a comprehensive benefits

Qualifications

• APP licensure and credentials required for the assigned service area.
• Minimum of five years of clinical APP experience in family/urgent/post-acute care.
• Strong communication, organization, care-coordination, and independent decision-making skills.
• Must obtain VA and NC licenses.
• Applicants may live in Indiana, Nevada, South Carolina, South Dakota, Tennessee, Texas, Virginia, West Virginia, or Wisconsin.

Responsibilities

• Review discharge records and complete focused assessments, medication reconciliation, and time-limited care plans.

Position summary

The Advanced Practice Provider will deliver timely, virtual care to eligible patients following hospital discharge. The APP will address immediate clinical and coordination needs, support a safe transition, and provide a clear handoff and/or bridge to primary care and other appropriate services.

Key responsibilities

• Review discharge records and complete focused assessments, medication reconciliation, and time-limited care plans.
• Manage appropriate lower-risk diagnoses and chronic conditions within program scope.
• Recognize red flags and elevate to emergency, specialty, in-person, or higher-acuity care using established protocols.
• Coordinate referrals and services, including home health, wound care, social work, behavioral health, pharmacy, and durable medical equipment.
• Complete timely documentation, continuity-of-care paperwork, and clear handoffs for tests, referrals, and follow-up needs.

Preferred qualifications

• APP licensure and credentials appropriate for the assigned service area.
• Minimum of five years of clinical experience as an APP in family medicine, emergency medicine, or long-term/post-acute care, including the management of medically complex adults and focused virtual assessments.
• Strong communication, organization, care-coordination, and independent decision-making skills.
• Will be required to obtain a VA and NC License

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $100,900.80 - $312,000.00 annually. Additional compensation may be available for this role, such as shift differentials, on-call pay, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
