Job summary

Sentara Health in Virginia Beach seeks an Advanced Practice Provider to deliver timely, virtual care to patients after hospital discharge. The APP will review records, perform focused assessments, reconcile medications, and create short-term care plans.

This remote, full-time role requires five years of APP experience in family or acute care, strong communication and care coordination skills, and the ability to work independently within protocols. VA/NC licenses may be required.

Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.
