Job summary

Community Partners in Action, Inc. is seeking a full-time Reintegration Mentor for the REGIONS-Site Secure Residential Treatment program for male youth in Hamden, CT.

The mentor will support trauma-informed care, family engagement, and coordinated transitions back to the community. The ideal candidate holds a bachelor’s degree in human services, psychology, or related field, has lived experience, and demonstrates strong emotional intelligence and leadership in a multi-cultural setting.

Qualifications

• Bachelor's degree in human services, psychology, criminal justice or related field; lived experience considered.
• Strong emotional intelligence and ability to engage with youth, families, and diverse populations.
• Knowledge of trauma-informed care, youth development, and family engagement preferred.

Responsibilities

• Collaborate with Clinician to engage youth, family and community resources for program admission.
• Facilitate youth and family team meetings and support transition to community after-care services.
• Reinforce each youth's individualized treatment plan and provide ongoing support to youth and family.
• Establish connections with community resources and vocational opportunities.
