Job summary

Elite Care Management, Inc. is seeking a compassionate Licensed Nurse (RN or LPN) for part-time home health care in Bloomingdale, IL.

You will provide one-on-one care for a single patient per shift, observe daily living activities, assist with care plans, and ensure patient comfort and well-being in the home setting. You will administer medications, monitor patient response, and coordinate with the physician and supervisor.

Qualifications

• Current RN or LPN license is required.
• Valid driver’s license is required.
• TB test results (2-step) or chest x-ray within 12 months.
• Current CPR certification.

Responsibilities

• Observe patients, record conditions, and notify supervisor or physician of changes.
• Follow organizational policies and procedures.
• Demonstrate strong spoken and written communication with patients and families.
• Manage and direct patient care according to the care plan and update as needed.
• Administer medications and treatments as prescribed and document effectiveness.
• Intervene in changes to patient condition and report to physician or supervisor.

You will get to know your patients and their families and learn about the needs they may have in their home. • Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
