Job summary

Elite Care Management is seeking skilled Registered Nurses or Licensed Practical Nurses for home health care in Illinois. Earn $35–$40 per hour with a sign-on bonus, full-time or part-time shifts, and one-on-one patient care in the patient’s home.

Responsibilities include administering meds, observing patients, and providing compassionate care to improve quality of life. Shifts include 7am–7pm on Saturdays, various weekdays/weekends, and 7pm–7am ending Saturday.

Qualifications

• RN or LPN license
• Current CPR Card
• TB Test Results (2-step) or chest x-ray within 12 months / QuantiFERON within 5 years
• Driver's License
• Social Security Card
• Auto Insurance Card
• State license for RN or LPN

Responsibilities

• Observe patient, record significant conditions and reactions, notify supervisor or physician
• Demonstrate understanding of nursing policies and procedures
• Demonstrate strong communication skills, both spoken and written
• Manage and direct patient care per the care plan and update as needed
• Administers medications and treatments as prescribed and monitors effectiveness
• Intervenes in response to changes in patient condition and reports to supervisor
