Elite Care Management, Inc. in Illinois (West Dundee) is seeking a part-time skilled nurse (RN or LPN) to provide focused, one-on-one home health care for a single patient during the 10am–10pm Saturday and Sunday shifts.

You will assess daily living activities, comfort, and overall well-being in the patient’s home. You will administer medications, follow the patient care plan, document observations, and coordinate with the supervising nurse.

Qualificações

• Observe patient, record significant conditions and reactions, and notify supervisor or physician of changes.
• Demonstrate understanding of nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.

Registered Nurse or Licensed Practical Nurse with Elite Care Management.
