Job summary

Elite Care Management, Inc. is seeking a part-time skilled nurse (RN or LPN) to provide one-on-one home health care in the patient’s residence.

You will work with a single patient per shift, building a detailed understanding of needs, coordinating care, and planning activities to improve comfort and quality of life. Responsibilities include observing patients, administering medications, following care plans, and communicating changes to the team.

Qualifications

• RN or LPN state license is required.
• Driver's license is required.
• CPR certification is required.
• TB test (2-step) or chest x-ray within 12 months, or QuantiFERON within 5 years.
• Social Security Card and Auto Insurance Card are required.

Responsibilities

• Observe patient and document conditions; notify supervisor/physician of changes.
• Follow nursing policies and procedures.
• Communicate clearly, in speech and writing.
• Manage patient care per plan and update as needed.
• Administer medications and monitor effectiveness.

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
