Job summary

Elite Care Management, Inc. is seeking part-time and PRN home health nurses to provide one-on-one care to patients in their homes in Wilmington. You will care for one patient per shift, plan, organize, and assist with home health care services under the patient care plan.

Responsibilities include observing patients, reporting changes, administering medications, and ensuring comfort and well-being. Licensing requirements include RN or LPN and CPR certification; valid driver’s license and

• Observe patients and report conditions to supervisor or physician.
• Demonstrate strong communication with patients and families.
• Manage and direct patient care per care plan and update as needed.
• Administer medications and monitor effectiveness.

Responsibilities

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
