Transitions Hospice is seeking an RN Case Manager in Canal Winchester, OH. This full-time nursing role requires coordinating patient care, guiding interdisciplinary teams, and ensuring high-quality end-of-life care in home settings.

Responsibilities include assessments, care planning, medication administration, and thorough documentation. On-call rotation and compassionate, independent patient support are core expectations.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans in collaboration with physicians and care teams.
• Administer medications and treatments per physician orders.
• Maintain accurate, timely, and meaningful clinical documentation.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers so they feel confident and supported.
• Participate in on-call rotation per team policy.

Schedule: Mon-Fri (8 hour shifts) with on-call responsibilities
