Job summary

Transitions Hospice in Midland, MI is seeking a Registered Nurse (RN) Case Manager to coordinate patient care, guide the interdisciplinary team, and provide compassionate, at-home hospice support. This full-time role offers a Monday-Friday schedule with on-call backup.

You will perform assessments, develop care plans with physicians, administer medications, document care accurately, and educate families. A state RN license and hospice or home health experience are preferred; you’ll join a team

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• 1+ year Hospice, home health or related organization experience preferred.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans in collaboration with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate, timely, and meaningful.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers so they feel confident and supported.
• Participate in on-call rotation per team policy.

Schedule: Monday - Friday (8am -5pm) with back-up on call requirements
