Transitions Hospice in Frankenmuth, MI, is seeking a Registered Nurse (RN) Case Manager for a full-time position. The RN will coordinate care, guide the interdisciplinary team, and support patients and families with medical and emotional needs in a home-based hospice setting.

The role requires an active RN license in the state, with 1+ year hospice or home health experience preferred, and strong communication skills for collaborating with physicians and care teams.

Coverage Area: Great Lakes Bay Region, Frankenmuth, Tuscola, Genesee, and Surrounding Counties

Salary: $40.00 - $46.00 / hour

Schedule: Monday - Friday (8am -5pm) with back-up on call requirements
