Transitions Hospice in Michigan is seeking an RN Case Manager to coordinate patient care in the Great Lakes Bay Region, Frankenmuth, and surrounding counties. You will partner with physicians and the interdisciplinary team to deliver compassionate, 24/7 support where patients feel safest—at home.

Your day includes comprehensive assessments, care plan adjustments, medication administration, documentation, and serving as the bridge between families and care teams.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsabilités

• Conduct comprehensive, ongoing assessments to understand each patient’s evolving needs.
• Build and adjust personalized care plans in collaboration with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate, timely, and meaningful.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers so they feel confident and supported.
• Participate in on-call rotation per team policy.

Coverage Territory: Great Lakes Bay Region, Frankenmuth, Tuscola, Genesee, and Surrounding Counties

Schedule:Mon-Friday (8am -5pm) with back-up on call requirements

Pay Range: $40.00 – $46.00 am hour
