Job summary

Select Home Health & Hospice is seeking a skilled Registered Nurse to provide in-home nursing care across central Indiana. You will perform patient assessments, implement physician plans of care, and educate patients and caregivers to promote independence and well-being.

The role requires Indiana RN licensure and at least 1 year of nursing experience in hospital, facility, or home health settings, with home health preferred. A valid driver’s license and reliable transportation are required.

Qualifications

• Graduate of an accredited school of professional nursing (required).
• Current Registered Nurse license in Indiana (required).
• Minimum of 1 year nursing experience in hospital, facility, or home health setting; home health experience preferred.

Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and complete required documentation (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications, disease processes, and in-home care.
