Job summary

Select Home Health & Hospice in Indiana seeks a compassionate Physical Therapist to provide skilled therapy in patients' homes, developing individualized care plans to support function, safety, and independence. You will work with patients and caregivers, delivering treatments and educating families on therapy goals and equipment.

The role requires graduation from an accredited PT program, state licensure, and a minimum of 1 year of PT experience.

Qualifications

• Graduate of an accredited physical therapy program (APTA or AMA approved).
• Licensed as a Physical Therapist in the state of practice.
• Minimum of 1 year of experience as a Physical Therapist.

Responsibilities

• Provide skilled physical therapy in patients' homes according to the physician's plan of care, seeing multiple patients per day.
• Evaluate and assess patient function and assist in developing and updating individualized care plans.
• Deliver treatments to relieve pain, restore function, and support maximum performance using appropriate modalities.
• Monitor patient progress, document outcomes, and communicate changes to the physician, DON, and care team.
• Educate patients, caregivers, and staff on therapy goals, techniques, and use of adaptive devices or equipment.
• Reassess patient needs regularly and revise plans of care as appropriate.
• Collaborate with interdisciplinary team members to ensure coordinated, high-quality care.
• Participate in quality improvement initiatives, staff meetings, and ongoing education.
• Provide required supervision of the Physical Therapy Assistant at least every 30 days.
