Job summary

Fortis Health is seeking a dedicated Registered Nurse to provide in-home nursing care in the West Lafayette, IN area. The role focuses on high-quality, personalized patient care while promoting independence and well-being.

Responsibilities include patient assessments, implementing care plans, educating patients and caregivers, coordinating services, and supervising LPNs/HHAs. Indiana RN licensure and valid TB test are required.

Qualifications

• Graduate of an accredited school of professional nursing (required).
• Current Registered Nurse license for the state of Indiana (required).
• Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience (preferred).
• Valid driver’s license, reliable auto insurance, and personal transportation (required).
• Current up to date TB test.

Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and complete required documentation (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications, disease processes, and in-home care.
• Coordinate patient services and supervise LPNs/HHAs as needed.
• Communicate changes in patient condition to physicians and care teams.
• Maintain compliance with regulations and agency policies.

Duties/Responsibilities

• Perform patient assessments and ongoing evaluations.

Physical Requirements
