Job summary

Select Home Health is seeking skilled and compassionate Registered Nurses to provide in-home nursing care in Avon, IN. This role emphasizes high-quality, personalized care while promoting independence and overall well-being.

You will perform patient assessments, implement medical plans, educate patients and caregivers, coordinate services, and supervise LPNs/HHAs. Strong clinical skills and reliable transportation are essential.

Qualifications

• Graduated from an accredited school of professional nursing.
• Indiana RN license required.
• Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience preferred.
• Valid driver’s license, reliable auto insurance, and personal transportation.
• Current TB test.

Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and document (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications, disease processes, and in-home care.
• Coordinate patient services and supervise LPNs/HHAs as needed.
• Communicate changes in patient condition to physicians and care teams.
• Maintain compliance with regulations and agency policies.

Job Summary

Duties/Responsibilities

• Perform patient assessments and ongoing evaluations.


• Implement physician-ordered plans of care and complete required documentation (OASIS).


• Provide in-home nursing care for multiple patients per day.


• Educate patients and caregivers on medications, disease processes, and in-home care.


• Coordinate patient services and supervise LPNs/HHAs as needed.


• Communicate changes in patient condition to physicians and care teams.


• Maintain compliance with regulations and agency policies.

Physical Requirements

• Ability to sit, stand, and drive for extended periods while providing in-home care.


• Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.


• Regular use of a tablet for documentation, communication, and training.
