Job summary

Select Home Health is seeking a skilled and compassionate Full-Time Registered Nurse to provide in-home nursing care in the West Lafayette area. The role emphasizes high-quality, personalized care while promoting independence and wellbeing.

Responsibilities include patient assessments, implementing care plans, patient education, care coordination, and supervision of support staff. Indiana RN license, valid driver's license, and TB test are required; home health experience preferred.

Qualifications

• Graduate of an accredited school of professional nursing (required).
• Current Registered Nurse license for the state of Indiana (required).
• Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience preferred.
• Valid driver’s license, reliable auto insurance, and personal transportation (required).
• Current up-to-date TB test.

Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and complete required documentation (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications, disease processes, and in-home care.
• Coordinate patient services and supervise LPNs/HHAs as needed.
• Communicate changes in patient condition to physicians and care teams.
• Maintain compliance with regulations and agency policies.

Physical Requirements
