Job summary

Select Home Health & Hospice is seeking a skilled PRN Registered Nurse to provide in-home nursing care across central Indiana. You will assess patients, implement care plans, document accurately, and educate families while maintaining independence and comfort at home.

Qualified candidates hold an Indiana RN license, BSN, and at least 1 year of nursing experience; driver’s license and TB test current are required. PRN shifts offer flexibility within a compassionate team.

Qualifications

• Graduate of an accredited nursing program and licensure in Indiana.
• Minimum 1 year nursing experience in hospital, facility, or home health setting.
• Must have valid driver’s license and transportation; TB test current.

Responsibilities

• Perform patient assessments and ongoing evaluations in home settings.
• Implement physician-ordered plans of care and complete documentation (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications and disease processes.
• Coordinate services and supervise LPNs/HHAs as needed.

Job Summary

Duties/Responsibilities

• Perform patient assessments and ongoing evaluations.

Physical Requirements

• Ability to sit, stand, and drive for extended periods while providing in-home care.
• Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.
• Regular use of a tablet for documentation, communication, and training.

Qualifications

• Current Registered Nurse license for the state of Indiana(required).
• Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience (preferred).
• Valid driver’s license, reliable auto insurance, and personal transportation (required).
• Current up to date TB test.
