Job summary

Valeo Home Health & Hospice provides in-home nursing care across the region, delivering compassionate services for patients in Weber, Davis, Salt Lake, and Utah counties. The part-time Registered Nurse will perform assessments, implement plans of care, and educate patients and families to promote independence and well‑being.

The role requires an active RN license in Utah, at least one year of nursing experience (home health preferred), and a valid driver’s license with reliable transportation.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license in the practicing state.
• Minimum 1 year nursing experience in hospital, facility, or home health setting (home health preferred).
• Valid driver’s license, reliable auto insurance, and personal transportation.
• Current TB test up to date.

Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and complete required documentation (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications, disease processes, and in-home care.
• Coordinate patient services and supervise LPNs/HHAs as needed.
• Communicate changes in patient condition to physicians and care teams.
• Maintain compliance with regulations and agency policies.
