Job summary

Select Home Health is seeking a PRN Registered Nurse to provide in-home nursing care across multiple patients in Monticello, IN.

Requirements: Indiana RN license, minimum 1 year hospital/facility/home health experience, valid driver's license and reliable transportation; TB test current.

Qualifications

• Graduate of an accredited school of professional nursing (required).
• Current Registered Nurse license for Indiana (required).
• Minimum of 1 year nursing experience in hospital, facility, or home health; home health preferred.

Job Summary

Select Home Health is seeking skilled and compassionate PRNRegistered Nursesto provide in-home nursing care. This role ensures patients receive high-quality, personalized care while promoting independence and overall well-being.

Duties/Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and complete required documentation (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications, disease processes, and in-home care.
• Coordinate patient services and supervise LPNs/HHAs as needed.

Physical Requirements

• Ability to sit, stand, and drive for extended periods while providing in-home care.
• Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.
• Regular use of a tablet for documentation, communication, and training.

Qualifications
