Job summary

Valeo Home Health & Hospice in Utah seeks an experienced Registered Nurse Case Manager to deliver in-home nursing care, assess patients, implement care plans, and educate families to promote independence and wellness.

The role requires an accredited nursing degree, an active RN license, at least 1 year of nursing experience, and a valid driver’s license. Travel to patient homes is expected; comprehensive benefits are offered.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license in the relevant state (Indiana listed in posting).
• Minimum 1 year nursing experience in hospital, facility or home health setting.
• Valid driver’s license and reliable transportation.
• Current up-to-date TB test.

Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered care plans and complete required documentation (OASIS).
• Provide in-home nursing care for multiple patients daily.
• Educate patients and caregivers on medications and disease processes.
• Coordinate patient services and supervise LPNs/HHAs as needed.
• Communicate changes in patient condition to physicians and care teams; ensure regulatory compliance.
