Elite Care Management, Inc. is seeking a Skilled Nurse (RN or LPN) for one-on-one home health care. You will care for a single patient per shift, assess needs, and plan, organize, and deliver services to improve comfort and well-being in the patient’s home.

The role offers a competitive hourly rate ($35–$40) with full-time and part-time options, paid orientation, and comprehensive benefits. Candidates must have active licensure and a valid driver’s license.

• 6am - 6pm - PRN - Various Weekdays & Weekends

Pay Range:

• $35 - $40 / hour

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
