Job summary

Elite Care Management, Inc. is seeking a per-diem Home Health Nurse (RN or LPN) to provide one-on-one care in patients’ homes in Riverwoods and surrounding areas.

You will care for a single patient per shift, coordinating daily living activities, comfort, and well-being under a patient care plan. Responsibilities include monitoring conditions, administering prescribed medications, and communicating changes to the supervisor or physician.

Qualifications

• Observe patient, record significant conditions and reactions, and notify supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding of organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, and monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
