Job summary

Elite Care Management, Inc. is seeking a licensed nurse for part-time home health care in Riverwoods. You will provide one-on-one care to a single patient per shift, building a close understanding of the patient and family needs while planning and delivering in-home care.

Responsibilities include observing patient conditions, administering medications, and following the care plan. Requirements include RN or LPN license, current CPR, and valid driver’s license along with TB/Chest X-ray or

• Driver’s license is required.
• RN or LPN state license is required.
• Current CPR card is required.
• TB test (2-step) or chest X-ray in last 12 months, or QuantiFERON gold within last 5 years.

Responsibilities

• Observe patient and record significant conditions and reactions; notify supervisor or physician as needed.
• Demonstrate ability to follow nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Manage and direct patient care per the care plan and update as needed.
• Administer medications and treatments as prescribed; monitor effectiveness.
• Intervene as needed and report changes to physician or supervisor.
