Job summary

Elite Care Management, Inc. is urgently seeking a part-time nurse to provide one-on-one home health care at Riverwoods. You will care for a single patient per shift, performing daily living activities and implementing the patient care plan with focus on comfort and well-being.

The ideal candidate holds an RN or LPN license, a valid driver’s license, and current CPR. We offer competitive pay, paid orientation, weekly pay, comprehensive benefits, and 24/7 on-call support.

Qualifications

• TB test results (2-step) or chest x-ray within 12 months or QuantiFERON gold within 5 years.

Responsibilities

• Observe patient, record significant conditions and notify supervisor or physician.
• Demonstrate understanding of nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Manage and direct patient care according to the patient care plan and update as needed.
• Administer medications and treatments as prescribed and monitor effectiveness.
• Intervene when patient condition changes and report to physician or supervisor.

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
