Elite Care Management, Inc. is seeking compassionate registered nurses or licensed practical nurses for home health care. You will provide one-on-one care to patients in their homes, monitor health status, and implement the patient care plan with the supervising clinician.

Positions available full-time or part-time, with shifts including day and overnight hours. Benefits package and on-call support are provided to support patient well-being and staff satisfaction.

Qualifications

• RN or LPN license in Illinois.
• CPR certification required.

• 7am - 7pm - Saturday
• 7am - 7pm - Various Weekdays & Weekends
• 7pm - 7am - e/o Saturday

Pay Range:

• $35 - $40 / hour

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents
• Demonstrate understanding organizational and nursing policies and procedures
• Demonstrate strong communication skills, both spoken and written
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor
