Job summary

Elite Care Management, Inc. offers home health care services delivering one-on-one patient care in the patient’s home. Nurses will provide comprehensive support for one patient per shift, focusing on comfort and well-being, with a strong emphasis on daily living activities and care planning.

Candidates must be a Registered Nurse or Licensed Practical Nurse; CPR may be required. Night shifts carry a $5/hour differential, with RN at $51/hour and LPN at $48/hour, plus benefits and weekly pay

Qualifications

• Current RN or LPN license is required.
• CPR certification preferred.
• Strong communication and documentation skills requested.

Responsibilities

• Observe patient and record conditions; notify supervisor/physician as needed.
• Follow nursing policies and procedures.
• Demonstrate effective communication, both written and spoken.
• Manage patient care according to the care plan and update as needed.
• Administer medications and monitor effectiveness; document results.
• Report changes in patient condition to physician or supervisor.

• 7pm - 7am - Friday / Saturday
• 7pm - 7am - PRN - Various Nights & Weekend Nights

Pay:

• LPN - $48 / hour
• RN - $51 / hour

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
