Job summary

Elite Care Management in Countryside, IL is seeking a dedicated home health nurse to provide one-on-one care to patients in their homes. You will work full-time or part-time, 7am–7pm on weekends, focusing on one patient per shift, planning and delivering home health services.

We offer a sign-on bonus, competitive pay of $35-$40/hour, weekly pay with direct deposit, comprehensive benefits, training, 24/4 on-call support, and referral programs. RN or LPN license, CPR, and TB testing are required.

• Strong communication skills, both spoken and written.
• Ability to manage and direct patient care according to the patient care plan.

Responsibilities

• Observe patient, record significant conditions and reactions, and notify supervisor or physician of patient status.
• Demonstrate understanding of organizational nursing policies and procedures.
• Demonstrate ability to manage and direct patient care according to the patient care plan.
• Administer medications and treatments as prescribed, and monitor effectiveness.

• Demonstrate understanding organizational and nursing policies and procedures

• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor
