Job summary

Elite Care Management is seeking qualified nurses to provide one-on-one home health care. You will work with a single patient per shift, observing, administering medications, and coordinating with care teams.

The role emphasizes personal bedside care, adherence to care plans, and comprehensive patient support in the home setting. Shifts include 7am–7pm or 7pm–7am, with sign-on bonuses for full-time and part-time roles.

Qualifications

• Licensed RN or LPN in IL
• Experience in home health care preferred
• Strong patient communication and documentation skills

Responsibilities

• Observe patients and report changes to supervisor or physician.
• Follow patient care plans and update as needed.
• Administer medications and monitor effectiveness.
• Document interventions and patient responses.

• $35 - $40 / hour

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
