Elite Care Management is seeking qualified nurses to provide one-on-one home health care. You will care for a single patient per shift, building a relationship with the patient and family while planning and delivering personalized home health services as a Registered Nurse or Licensed Practical Nurse.

Qualifications

• RN or LPN license required and in good standing.

Responsibilities

• Observe patient, record significant conditions and notify supervisor or physician of changes.
• Demonstrate understanding of nursing policies and procedures.
• Demonstrate strong spoken and written communication.
• Manage and direct patient care per the care plan and update as needed.
• Administer medications and treatments as prescribed and document outcomes.
• Intervene as needed and report changes to physician or supervisor.

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.


• Demonstrate understanding organizational and nursing policies and procedures.


• Demonstrate strong communication skills, both spoken and written.


• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.


• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.


• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
