Job summary

Elite Care Management, Inc. is hiring a Registered Nurse or Licensed Practical Nurse for home health care in Chicago. You will provide one-on-one care to a single patient per shift, supporting daily living activities and well-being.

Shifts include 7am-7pm, 7pm-7am, and PRN. Full-time and part-time roles with a $10,000 sign-on bonus and comprehensive benefits. We offer orientation, patient training, weekly pay, and a complete benefits package including medical, dental, vision, 401(k), PTO and

Qualifications

• Current CPR Card
• RN or LPN state license
• Driver's License
• Social Security Card
• Auto Insurance Card
• TB Test Results - 2 step or chest X-ray within the last 12 months OR QuantiFERON gold blood work within the last 5 years

Responsibilities

• Observe patient, record significant conditions and reactions, and notify supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents
• Demonstrate understanding of organizational and nursing policies and procedures
• Demonstrate strong communication skills, both spoken and written
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor
