Job summary

Elite Care Management, Inc. seeks a licensed nurse (RN or LPN) for home health care assignments. You will provide one-on-one care in patients' homes, covering full shifts for designated days, with a focus on comfort and well-being.

Shift hours are 7am–7pm on weekends, with option for on-call support. We offer a competitive salary, a sign-on bonus of $5,000 for full-time and $2,500 for part-time, paid orientation, and a comprehensive benefits package including medical, dental, vision, 401K, PTO,

Qualifications

• RN or LPN license required and in good standing.
• CPR certification is current and valid.
• TB test results within required timeframe or QuantiFERON test.

Responsibilities

• Observe patient and report significant changes to supervisor or physician.
• Follow nursing policies and procedures and maintain documentation.
• Deliver patient care per care plan and update as needed.
• Administer medications and document outcomes.
• Respond to patient condition changes and inform supervising physician.

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents
• Demonstrate understanding organizational and nursing policies and procedures
• Demonstrate strong communication skills, both spoken and written
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor
