Job summary

Ascension St. John’s Medical Center in Tulsa, OK is seeking a seasoned RN Case Manager to lead, plan, and coordinate patient care from admission through discharge.

This full-time role supports the Case Management department and integrates with discharge planning to optimize patient outcomes. The ideal candidate holds an active OK RN license, with 3–5 years in case management and a track record of improving workflows and reducing readmissions.

Qualifications

• Licensed Registered Nurse credentialed from the Oklahoma Board of Nursing.
• Case Manager credentialed from the American Case Management Association (ACMA) preferred.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
• HS diploma/GED with 2 years of experience or an Associate's/Technical degree accepted.

Responsibilities

• Mentor Colleagues: Help junior case managers with complex cases and prioritizing their tasks.
• Improve Processes: Review case management data to identify trends and enhance workflows.
• Prevent Readmissions: Develop detailed discharge plans to lower 30-day readmission rates.
• Ensure Continuity of Care: Support patients from admission through discharge to prevent care gaps.

Schedule: Full-Time, Monday-Friday, 8am-4:30pm

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• Licensed Registered Nurse credentialed from the Oklahoma Board of Nursing obtained prior to hire date or job transfer date required.
• Case Manager credentialed from the American Case Management Association (ACMA) preferred.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education

• High school diploma/GED with 2 years of experience, OR Associate's degree, OR Technical degree

Additional Preferences
